Work experience Account Representative
Develop relationships with referral sources including hospital social workers, case managers, physicians, subacute rehabs and Assisted Living facilities in Cumberland, Salem and Gloucester counties.
Meet with potential hospice patients and caregivers to asses appropriateness for hospice. Educate on hospice care and if appropriate, coordinate discharge with hospital staff.
Develop relationships and market to hospital and rehab facility case managers and social workers.
Meet and qualify patients who will discharge with Apria services including Oxygen, CPAP, Enteral feedings and negative pressure wound therapy.
