As a care coordinator I work with young people to help develop a service plan and follow through with the service plan. I assisted in helping clients find a public defender, hands on learning projects to help develop personal goals and was responsible for all medical appointments. As a care coordinator I was responsible for assessment documentation and working with there supports to help them find community and build out their support systems

As a case manager I work with clients to help develop a service plan and follow through with the service plan. As a case manager I was responsible for treatment team documentation and working with there teams including New Day wrap around service.

I monitor residents, provide them with guidance and support through my nurtured heart training. I work closely with case managers and clinicians to make sure their needs are met and they are able to succeed in our 3 months program. I monitor them during outings and assist with medication. I teach the life skills and help with projects and homework the residents might be working on during their stay.
