I am currently serving as a Human Services Coordinator with the Division of Developmental Disabilities. My responsibilities include supporting members and their families in accessing the resources they need to lead independent and fulfilling lives. I diligently document notes during each meeting to ensure that the members' needs are properly addressed. I also encourage the participation of all team members in these meetings.
My duties encompass assessing service needs such as habilitation, respite, and attendant care services. I update and focus on each case, assigning therapies to the appropriate vendors. Additionally, I document all encounters with families in Prognote, respond to emails and phone calls promptly, and coordinate crisis interventions when necessary. I also facilitate the setup of behavioral health services tailored to each member’s individual requirements and promote community involvement.
Furthermore, I participate in all huddles, unit meetings, and team discussions to stay aligned with the team’s objectives and best support the members we serve.
I have had the opportunity to work with both verbal and non-verbal individuals, and I am familiar with communication devices that individuals may use to express their wants, needs, and feelings. I am committed to advocating for those who may have difficulty speaking and strive to communicate effectively with every person I serve.

In my past employment experience, I have had the privilege of working with adults with special needs across a variety of abilities. I am dedicated to supporting them with their daily living skills and personal goals, always approaching their needs with respect and sensitivity. I assisted with dressing and incontinence care as needed, and I prepare meals while offering support during mealtimes if required. My experience includes working with G-tube feedings, administering nutritional formulas, medications, and water, ensuring their well-being.
