• Coordinates with the case managers to provide Independent Living services to youth served by the Permanency Program.
• Assesses eligibility for youth in Extended Care program to include youth in Trial Independence who request to return to paid care.
• Ensure completion of the CLSA for all youth 14 and over and any other assessments needed to prepare youth for independent living.
• Maintains sufficient case documentation including forms and narratives to provide a complete and accurate written record in assigned Preparation for Adult Living only cases for each youth receiving Preparation for Adult Living services.
• Works for Permanency Specialist to ensure youth in care are receiving counseling and guidance in preparation for independent living/emancipation while they are still in care.
• Participates in case planning conferences as needed.
• Compiles monthly statistical data and reports and forwards it to appropriate agencies.
• Completes documentation and forwards reports containing descriptive, analytical, and evaluative content.

• Provide watchful care in helping to apply behavioral management, service plans, safety plans, behavior plans, and response plans
• Complete accurate and detailed documentation daily and perform administrative functions in relation to reporting and documenting
• Attend weekly staff meetings to provide thought-input and feedback to advocate for the participant in relation to service plans for each participants programming and performance
• Utilize the strength-based approach in providing feedback for participants
• Engage with trauma-informed care techniques, behavioral management, and advocate for the participant to meet organizational goals and individual participant goals

• Assist in gathering information about basic and community living of the person receiving services to be used in the assessment process; implement designated interventions and behavioral management programs
• Communicates regularly with clients and engages clients to talk about their mental illness and substance misuse
• Assists clients in preparing for court appearances or other outside activities, and implements supportive, psycho-educational and recreational groups
• Oversees collection of urine specimen for drug screens and may complete and record vital signs as assigned
• Assists clients in gathering/obtaining personal property when discharged
• Answers phones and provides other administrative support as needed
• Builds new case files and breaks down discharged client case files

• provides programs and services for adolescents and adults who face significant emotional, behavioral and intellectual/developmental challenges.
• Supporting a therapeutic environment, while ensuring the safety and well-being of the individuals.
• Following and implementing behavioral support plans.
• Documenting shift reports, incident reports, and individual progress notes.
