• Maintained an active intensive changing caseload of individuals diagnosed with: mental health disorders ; and dual-diagnosed with a mental illness with substance abuse disorders or a developmental disability.
• Completed service & coordination plans, progress notes, and DMH required documentation within 2-3 days.
• Restored or maintained a client's functional , daily living & social skills at home or in the community to promote stabilization in various settings.
• Linked clients and families to resources : housing,employment, transportation, medical, SSI/SSDI benefits and other resources needed.
