Create and communicate work schedules staff meetings for the department and other programs
Plans, coordinates and facilities, meetings and events.
Conducts and supports staff with client meetings, Multiple Disciplinary Teams, Trauma Informed education and Motivation Interviewing continued education.
Ensure staff are up to date on policy and procedures and company training and standards
Assume primary responsibility for assisting homeless clients to develop, implement, evaluate and revise overall case plan goals and objectives as they relate to the attainment/ maintenance of income, increasing savings, and moving into permanent housing.
Actively participate as a member of a multidisciplinary team serving a group of homeless clients, including preparing and presenting client case presentations
Provide clients with ongoing support services, which include assessment, evaluation, coordinating services, crisis intervention, referral to community resources, and documentation.
Assessment includes client interviews, multidisciplinary team coordination, approved tests and assessments, medical and legal records, and professional collateral communications, to develop a case plan that reflects each client’s unique circumstances.
Provide problem solving, role-modeling, and skills training to assist clients to gain or maintain income, build savings, and obtain permanent housing.
Prepare and provide action of acquiring medical and mental health records to build and submit SSI/SDI claims through the HOPE/SOAR Process 2016- 2018.
Complete finance client rent and property management spreadsheets and check /ACH request.
Complete petty cash approvals for staff for client activities and assorted financial requests.
Correspond with Property Managers/Owners to correct any concerns or problems with rent payments.
