• Using the CREED Companionate, Respect, Empathy, Empowerment and Dignitary. Utilizing wrap around services to ensure all angles are approached to ensure clients can be successful to self-sufficiency. Approach, to provide services to clients of a non-profit agency into permanent housing and to identify their needs and direct them to the appropriate services, internally and in the community for resources to ensure their success of income, savings, housing and self-sufficiency. Assist and support clients with Mental Health, Substance Abuse and Co-Accusing challenges and Disabilities.
• Essential Functions:
• Responsible for 6 FTE and 1 PTE
• Tracks expenses for the department.
• Conducts Training, assigns tasks and monitors work performance and engages and retains a highly performing accountable workforce.
• Monitors and evaluate the work productivity of staff to include policies, procedures, caseloads, client interactions, interventions and documentation of services
• Create and communicate work schedules staff meetings for the department and other programs
• Plans, coordinates and facilities, meetings and events.
• Conducts and supports staff with client meetings, Multiple Disciplinary Teams, Trauma Informed education and Motivation Interviewing continued education.
• Ensure staff are up to date on policy and procedures and company training and standards
• Assume primary responsibility for assisting homeless clients to develop, implement, evaluate and revise overall case plan goals and objectives as they relate to the attainment/ maintenance of income, increasing savings, and moving into permanent housing.
• Actively participate as a member of a multidisciplinary team serving a group of homeless clients, including preparing and presenting client case presentations
• Provide clients with ongoing support services, which include assessment, evaluation, coordinating services, crisis intervention, referral to community resources, and documentation.
• Assessment includes client interviews, multidisciplinary team coordination, approved tests and assessments, medical and legal records, and professional collateral communications, to develop a case plan that reflects each client’s unique circumstances.
• Provide problem solving, role-modeling, and skills training to assist clients to gain or maintain income, build savings, and obtain permanent housing.
• Prepare and provide action of acquiring medical and mental health records to build and submit SSI/SDI claims through the HOPE/SOAR Process 2016- 2018.
• Complete finance client rent and property management spreadsheets and check /ACH request.
• Complete petty cash approvals for staff for client activities and assorted financial requests.
• Correspond with Property Managers/Owners to correct any concerns or problems with rent payments.

• Using a "whole person" approach, to provide services to clients of a non-profit agency to identify their needs and direct them to the appropriate services, internally and in the community. Focusing on hard to serve, vulnerable, disadvantaged veterans and OEF/OIF veterans.
• Responsible for six FTE and three PTE, as Acting Director, in charge up to and not limited to 13 FTE and 5 PTE.
• Responsible for hiring, quarterly reviews, coaching, mentoring and training.
• Continuum Case Management of clients by monitoring, directing, and coaching for self-sufficiency.
• Train, assist, coach and guide new case managers of internal procedures, policies and daily flow of clients and grants.
• Acting Director of facility, Transitional Housing, Alcohol and Drug Treatment Program, Assessment Center and Employment Center in the absence of Directors of the program. Support Directors in day to day operations, grant preparations, and implementation of on-call support
• Responsible for multiple grants, HVRP, VEAP, PRI, Acting responsibility for HUD, SASMA and smaller grants, involved and limited to writing, , State and Federal OMB Circular, Reporting, and adhering to required guidelines, outcomes and monitoring.
• Ensuring Evidenced Base Practices were followed, Psychosocial Assessments, Individual Service Plans (ISP), Counseling– i.e. Alcohol and Drug Treatment, Behavioral Health, Homelessness, Transitional Housing, Medical /PTSD, Employment and Education. Counseling addressing, mental health, alcohol and drug, housing, employment, education and legal.
• Career planning, employment counseling, employment training and instructional programs
• Close collaboration with VA Representative, Veteran Service Officers, and VA Social Workers. VA Medical Systems
• Close collaboration with State and Federal Representatives and local municipalities.
• Presentations at N. California prisons and correspondence with internal parole agents for preparation of inmates release and reintegration into society.
• Correspondence in person, phone and email with local Dept of Corrections personnel, PO Agents, Public Defender
