• Provide community and early intervention outreach services, establish resources for clients and their families, provide coordination to obtain services, remove barriers, facilitate admission, retention, completion of treatment and follow through w/Continuing Recovery activities.
• Contact community based organizations and providers to assist in coordination of services, encourage participation in treatment, assist clients and their children in gaining support services, and provide support to Clients and referring agencies.
• Conduct Person-Centered, strength-based assessments for substance use disorders to identify goals and establish a therapeutic alliance, provide accurate substance use disorder diagnoses based on DSM-5 criteria, complete an Assessment Summary, determine an appropriate level of care using current ASAM criteria, assess the need for referrals to address immediate medical or mental health issues, and the need for a referral.

• Provide person-centered support, recovery planning, and care coordination while assisting clients in developing their Initial Treatment Plan and Continuing Recovery Plan. Be a resource to clients in identifying treatment methods and meeting recovery goals. Provide care coordination, crisis intervention, update Treatment and Continuing Recovery Plans, progress and other reports to DCFS, DHS, TASC, courts, probation, employers, etc., and provide utilization review updates to insurance.
• Provide person-centered counseling and education services, conduct individual counseling sessions, Facilitate groups, develop and present educational materials, and provide family sessions,

• Handle incoming calls, provide clients with paperwork, maintain all paperwork, provide referrals to individuals who are ineligible to receive services, complete Initial Screenings, advise clients of interim group, forward to Client Accounts, maintain wait lists, and collect random drug screens.
• Confirm appointments, maintain schedule, arrange transportation, verify and/or acquire medication authorizations, follow Concurrent Utilization Review procedure for readmission, contact and encourage family members to attend family day, organize, distribute, and maintain supply of all admissions paperwork, enter and admit data and payer sources into Client Tracking, update all admission spreadsheets daily, and provide Crisis Intervention services.

• Monitors client activity, Facilitate completion of client urine drug screens / breathalyzer testing per procedure, Conducts new client orientation, provide Crisis Intervention services, develop and present educational materials utilizing individual assignments and lectures on a variety of recovery-based topics. monitor entrance into the building outside of regular business hours. observe interactions among clientsto ensure clients are not engaged in any unhealthy behavior. Using the discharge date from the Client Continuing Recovery Plan schedule, send out follow-up survey letters and Complete initial, transfer, ongoing and discharge Quantitative Record reviews.
