• Coordinate referrals coming from Physician offices, Hospitals and Facilities, provide Hospice education on services and explain Hospice philosophy to families, order DME equipment as needed, utilize a CRM system for daily tracking through Trella Health and Playmaker Sparks, Community public relation events and public speaking, marketing for program expansion in Schuylkill, Berks, Lebanon and surrounding counties, and responsible for tracking admissions and referral reports, EMR systems Kinser and Matrix.

*Responsible for marketing Home Health, Hospice and Private Duty Services to Skilled Nursing facilities, Physician Groups, Independent Living and Assisted Living.

*Provided community education on BVNA services and Disease Management, coordinated ALL referrals into admissions for HH, Hospice and Private Duty.

*Ran Admissions and Census reports daily.

Worked in EMR systems such as Mckesson, Homecare Homebase, and Matix.

* Established and maintained effective communication with staff, physicians and community organizations to promote high quality patient care.

• Responsible for discharge planning and arranged Continuing Care for patients when being discharged from inpatient setting, completed follow-up calls on aftercare on patients leaving treatment going into Outpatient, Intensive Outpatient , Halfway housing or Extended Care Treatment, ran groups in schools on Tobacco Cessation, Drug and Alcohol Insight facilitating groups in Berks, Lehigh, Lebanon and Schuylkill counties, also worked as a Case manager with a team of therapist coordination patient care
• Am Certified as a Student Assistance Trainer in 2003.
