• Responsible for developing relationships with members of the community to engage Medicare seniors with Oak Street Health through event execution and local community outreach

• Promoted to Lead Case Manager in September 2015. In addition to maintaining a full caseload of both home and community-based, as well as facility-based members, assisted supervisors in planning and implementing training for new staff
• Served as a mentor and preceptor to case managers. Additional duties included shadowing/reverse shadowing case managers on member visits to ensure compliance, and participating on various committees

• Managed a caseload of 60+ home and community-based members for a Medicaid Long-Term Care program with very minimal supervision
• Educated members and families about program services/requirements and ensured member involvement in developing a plan of care
• Tasks included performing face-to-face quarterly visits, completing authorizations for providers, and ensuring that other contractual obligations were met

Served as a Case Manager under the Florida Medicaid Nursing Home Diversion program. Department was eliminated when the company was not awarded a contract as one of the Medicaid Managed Care Organization plans in the state of Florida.

• Employed as an Aging in Place Specialist, facilitating a Medicaid Waiver Nursing Home Transition program and a specialized amplified telephone program for the hearing impaired (FTRI)
• Performed marketing functions for these programs, as well as the organization as a whole, which included attending networking meetings, and organizing and promoting a health fair for seniors and people with disabilities.
