At Diversity Health Services, a provider agency, I worked with children, the elderly and the MRDD population. My job duties included:

Coordinating with client's support coordinator to formulate a personalized plan of care that addressed each client's daily living needs thereby ensuring that they could continue to live a successful and full-filled life. in the community. I supervised 10-20 Direct Support Workers in the home to ensure that client's had qualified competent staff. I completed client home visits to maintain continuity of services, and I trained prospective new DSWs.
