Works with MAP/MAP Basic/SFS pts that are complex/non-compliant with their own healthcare needs, assists pts in coordinating their care such as appts, specialty appts, transportation coordination with Lyft or medical management transportation, manages their diabetes, blood pressure, assisting pts with ESRD getting access to traditional dialysis program, medication adherence/social determinants issues, and assist pts to meet their healthcare goals.
