All leaves will be applied concurrently, unless otherwise noted above. If your leave time is intermittent or reduced schedule, any absences reported will be counted against your leave entitlement. Please see your Notice of Rights and Responsibilities form for information about your eligibility. Denials: If your leave of absence or accommodation was denied, you are eligible to apply again in the future. Medical documentation received after the date of this denial notice will typically be reviewed forward from the date received. Absences you incurred while your leave request was under review may be subject to the attendance policy. Please contact your Human Resources department if you have any questions. Accommodations: If you have a working accommodation, it may be necessary for you to openly discuss your restrictions and physical limitations with your supervisor to ensure your safety and adherence to your restrictions. Parental Leaves: Leaves related to the birth of a child will be approved starting on the estimated date of delivery. Your approval date will be adjusted based on the actual date of delivery. Unless you are taken off due to medical necessity prior to the birth, your leave of absence will not start until the baby arrives. Intermittent Approvals: If you are approved for intermittent time, it is your responsibility to keep track of your available time, but you can request a record of your time used from Flores Leave Solutions using the contact information below. Benefit Deductions: You will be required to pay for any missed benefit deductions upon returning from your leave of absence. If payments are not made timely, your group health insurance may be cancelled, provided that your employer notifies you in writing at least 15 days before the date that your health coverage will lapse. If your employment is terminated, you will still be responsible for paying the benefit premiums in full. Please reach out to Human Resources for additional questions. Changes to Leave Dates: Please notify us as soon as practicable if there are changes to your schedule leave dates or your leave is extended. Paid Time Off: You will be required to exhaust your PTO for the portion of your leave that is not covered by FMLA. For time covered under WI FMLA, you may choose to use PTO or take the time unpaid. Any paid time will run concurrently with your leave entitlements. For Paid Parental Leaves, PTO does not apply for the time covered under the company policy. Paid Leave Coordination: Any paid leave will run concurrently with your protected leave entitlements. Return to Work: You may be required to present a release from your doctor prior to returning to work. A Fitness for Duty form is included and should be completed by your health care provider and returned to Flores Leave Solutions prior to your return to work. You must check in with HR prior to returning to the floor in your first day back to work.
