  • I handle a case load of patients and their insurance needs
  • I transition them to the next level of care, SNF, inpatient rehab, and home health.
  • I conduct meetings weekly with my team to determine the status of each patient

  • I assist in setting goals for each individual patient, so they have the best outcome.
  • I handle all insurance issues for the patient including: Commercial Continued Stay, Medicare length of stay, Medicare Advantage plans, denials, and approval for each patient.

  • I conducted family meetings alongside the doctors when needed for Hospice consults, end of life measures.

  • I handled all the skilled nursing patients and their discharges.
  • Many other meetings and processes went into this job as well, Medicare meetings with the care team, care coordination meetings weekly with patients families to ensure the patients well being was taking care of.

  • I was also the key holder of the preschool when the administrator was out.

  • I held special meetings for any kid and their parents that needed additional time for questions.

  • I worked at Sparks for a year and a half as a Social Worker for the hospital.
  • I managed patients and where they went for the continuation of their care, or their transition home with home health.
  • I set up and Medical equipment needed in the home for the patient to be successful.
  • I also worked with the court system on any DHS/APS referrala that were made.
  • I committed patients at the prosecuting attorneys office when the doctor deemed it necessary. I then followed up with it in the court room on the date of the hearing.
  • Typical case load was 3 floors including the mother baby floor and NICU.
  • I also did extensive computer work with documentation.

My Name is Laney Spears. I went through the Social Work Program at Northeastern State University. I graduated in December of 2014. I got a job at Sparks Hospital in Fort Smith, AR.
