• Conduct twice monthly home visits with youth and foster parent
• Collecting maintaining case records
• Meeting monthly documentation deadlines
• Writing Case Plans (7 day, 30 day, 6 month & Annually)
• Complete ILP activities related to children 14 & up
• Knowledge of community resources and services
• Assist with completion of policy violations & Incident reports as necessary.
• Monitoring child's safety, permanency and well-being through twice monthly
home visits and frequent involvement with the child, foster family and DFCS Case
Manager.
• Support foster placement stability through monthly visits, coaching foster parent(s)
service coordination, treatment planning and crisis intervention.
• Advocate for each child's best interest by attending court hearings, panel reviews,
educational meetings, etc.
• Assisting with Investigating Policy Violation Assessments & completing relevant
documentation.
• On call schedule & Crisis management & support to foster parents
• Data Entry into GA SHINES- Monthly Contacts & RBWO Summaries
