Evaluate applications and determine eligibility for various social service programs such as Medicaid, Medicare, SNAP (Supplemental Nutrition Assistance Program), TANF (Temporary Assistance for Needy Families), etc
Ø Developed individual case plans based on client’s needs and eligibility using Salesforce & ASPEN.
Ø Managed the progress of each case ensuring that clients received the services they need in a timely manner.
Ø Verified and updated the required documents from clients such as proof of income, identification, residency and medical records.
