Assists clients in applying for Nutrition and Medical Assistance through DES. Assists client in applying for SSI and SSDI. Communicates with people from various cultures and backgrounds on application process.

Conducts home visits to clients, prepares for and designs Peer Support Groups. Client follow-up. Utilizes Microsoft Office and other resources for meetings and running Peer Support groups at an SMI Clinic.

Traveled to clients' homes to provide healthcare services and promote continuity of care. Provided patients and families with emotional support and instruction in preparing healthy meals, independent living and adaptation to disability or illness.

Duties included monitoring patients, vitals, holding groups, mentoring, one-on-one attention, meals, computer notes, and face-to-face meetings with mental health clients.

Worked with computerized and automated systems for dispensing medications and editing new patient profiles.
