Lead Caregiver

Freedom Home Care
08.2022 - Current

As a Lead Caregiver, I am able to operate in an office space and in elderly care. I manage the front desk and handle administrative projects when I am in the office, otherwise I am in the clients house helping them stay as independent as possible without moving out of their living space.

Experience working in an office has taught me how to collaborate with multidisciplinary teams to provide holistic care and address the physical, emotional, and psychological needs of each client.

  • Collaboration with healthcare professionals has developed comprehensive care plans tailored to each client's specific requirements.
  • Providing proper phone etiquette implementing compassion and patient-focused care to further client, caregiver, or other party to cultivate well-being. Managing over 20 incoming calls, messages, and mailing per day from clients, caregivers, and other office team members.
  • Organize spreadsheets, inventory, and audits by updating client or caregiver profiles, cross referencing data and scanning and uploading documents to ensure accurate record-keeping.

When I am out in the field working in the client's home, I am assisting clients with daily living needs to maintain self-esteem and general wellness. I am entrusted to handle confidential and sensitive situations in a professional matter. I am able to support elderly care in multiple different aspects like supporting:

  • Daily hygiene needs of patients by assisting with bathing, dressing, dental care and personal grooming.
  • Mental support and physical activities to accomplish quality of life and sustain needs.
  • Clients in meaningful conversation, socialization, and activity while providing personal care assistance.
  • Mentored junior caregivers, helping them develop essential skills and confident decision-making abilities in high-pressure situations by facilitating smooth transitions between shifts with prepared detailed handover notes outlining critical information about each patient's status.

Youth Counselor

Leo A. Hoffman Center
01.2022 - 07.2022

As a Youth Counselor, I had the opportunity to work with male adolescents in an in-care facility with other youth counselors, therapists, activity directors, and teachers by:

  • Connecting clients with appropriate external resources when needed, ensuring continuity of care beyond counseling program scope.
  • Implementing creative therapeutic interventions tailored to individual needs, promoting healthy expression of emotions.
  • Developing goal-oriented psychoeducational and activity therapy groups to improve clients' level of functioning.

I was able to learn how to create and implement individualized plans for individual youth by listening to clients' concerns and provided encouragement and support. I supervised meetings with family members to help create support system for clients, facilitated goal-setting workshops for positive future planning, supporting academic and personal achievements, and guided clients in effective therapeutic exercises integrated from Cognitive Behavior Therapy and Dialectical Behavior Therapy (DBT).

I assessed patient behavior, moods, and other responses to determine correct diagnosis and devise treatment plans, documented detailed case notes efficiently, maintaining confidentiality while adhering to reporting protocols set forth by the organization. Developed strong rapport with youths, fostering open communication and trust for effective counseling sessions.
