Providing case management services to veterans in need of housing assistance, re-location resources, and eviction prevention services. Veterans and case manager develop service plan, assessment completed at time of intake into program. Under either rapid rehousing or eviction prevention services the enrollment is initially 90 days-180 days. CM also responsible for establishing long term housing plan for clients; referring to either Shadow Subsidy (24 month rental subsidy program) or HUD VASH. Case manager connects veterans to other community resources to provide additional supports and assistance to promote housing stability. CM works as liaison with landlord/ management companies on behalf of the veteran to mediate, promote, and advocate for the veteran. CM refers and works with internal team to provide housing resources, employment services and healthcare navigation needs.
Property Insurance specialist

Provide intense case management services to clients suffering from severe and persistent mental illness. Provided wrap around services such as housing, employment, educational/ academic support and resources to maintain clients in the community. Managed finances for a caseload of 10-15 clients. The primary liaison for placement with all outside agencies such as department of rehabilitation services and specialized vocational training, and Associate degree programs with local community colleges.
