Community Health Partnership, Colorado Springs, CO, 09/01/13, 08/31/15, Community Health Partnership working remotely inside the Emergency department Of Penrose Main and Memorial Central Hospitals. Meeting with each patient receiving Medicaid services, tracking trends and utilization of services in our system to identify whether further resources or a care management team was needed, CARES program.
