- Place PCCP (Personal Care Plan) and HCBS (Home and Community-Based Services) referrals, ensuring clients receive appropriate care services.
- Monitor timestamps and ensure adherence to state policy.

- Oversee and maintain accurate and compliant resident files per DMH and DHSS regulations.
- Ensure employee records are fully compliant with DMH and DHSS guidelines.
- Coordinate resident appointments and arrange transportation, ensuring punctuality for all appointments.
- Communicate with residents' physicians to verify care plans are being followed and address any medical concerns.
